Updated to [pandocmeta](https://github.com/odkr/pandocmeta.lua) v0.4-0.

pandocmeta` failed to handle some YAML headers properly. This release uses a newer version that fixes those problems.

Fixed an rSt syntax error in the README.

* Minor code corrections.
* Removed manual page.
